[help] Associate SNP with Disease

Good Afternoon !!

I'm looking for a solution (any db) that links SNP with Disease.
I've been looking for it at NCBI, but my results are only the SNPID that is associated, but i really need a list with at least the name of the Associated disease..
something like this :

SNP:rs000111 SNPName:name SNPDesiase: name of Disease or some ID

does someone have it or knows how to get it ?

There are many places that have what you asking for, one is HGMD the human gene mutation database, it not free. And there are other LSDB's locus specific databases here is directory (http://www.hgvs.org/dblist/glsdb.html) depending on disease/gene of interest. NCBI has a new varview which may contain clinical information for your gene (http://www.ncbi.nlm.nih.gov/gene/672) is an example. Hope this helps.
