Homebrew End-Repair?

Does anyone have a homebrew protocol for end-repair? I know that most commercial kits contain Klenow fragment, T4 DNA polymerase, and T4 PNK. I've also seen this referenced in various papers, but have been unable to figure out the exact ratio of the mix. Any help would be greatly appreciated.
